NUR 567 Adv Acute Care Discharge and Health

The focus of this course is on discharge planning from acute care settings. The course will explore the role of the advanced practice nurse serving as a patient advocate and collaborating within systems to facilitate health promotion and the transition from acute care settings to community settings.
